Adding required attachments
Hello,
We have several required attachments that need to be submitted with our applications, however I'm not sure where to do this for the majority of them. MOUs can be uploaded on the Project Profile page and Resumes/CVs on the Key Personnel page. Where do we upload other required documents such as the W9, insurance documentation, data outcomes, etc.? It looks like our non-profit status can be uploaded on the Organization Details page, but I'm not sure that's the right place to be included with the application.

Hi Karen,
You can upload the W9, insurance documentation, etc. on the " Project Budget Summary" page. If you scroll to the bottom of the page there you upload any required documentation as outlined in the RFA there that did not have a specific space to upload them elsewhere in the application.
Just a note: data outcomes should not be uploaded in this spot, it will be upload in the "Project Work Plan- Objectives" section. This is a document we created that you will need to report on if funded, but because GeMS makes you upload sample evaluations forms/surveys as part of the application we gave this to applicants to upload in so you do not have to create a special one.
And yes, you can upload your non-profit status paperwork on the "Organization Details" page.

Another note on attaching numerous documents
Please note that you can also scan several documents to create one big attachment to upload- or copy/paste documents 'end to end' and save as a pdf to make one long document consisting of everything that you want to attach.
Again, the best place to upload additional docs or docs that are not required as attachments in other sections of geMS is at the bottom of the budget summary section but make sure you save the Komen Denver Data Outcomes excel spreadsheet to upload as an evaluation sample in the Project work plan section- you must upload something in this section or you will not be able to submit your application.

One more note about attachments!
Please be aware that the upload field on the project budget summary page is intended as an area to collect any remaining documents that are required in the RFA that do not have a specific place for upload (such as the 990).
If you add other documents that are not required anywhere else in the RFA into this area, they should not include additional narrative above and beyond what you fill in on the GeMS application pages. Any such documents will not be provided to the review panel and not considered in the review process.
The items listed in GeMS are only meant to be examples so please refer to your specific RFA for the required documents you need to provide!
