#9 in the Treatment RFA states "you MAY upload a copy of the Data Outcomes Sheet....." Do you want the Data Outcomes Spreadsheet with the organization name to establish the format for reporting--without data? Please expand on your intent for
this as an attachment at this stage. Many thanks.

Great question!

When you fill out the project plan pages (goals, objectives, etc...) on GeMS you have to upload something in the evaluation section of the project work plan. The system will not allow you to save your goals if you do not upload something in the evaluation area.

You may upload the Data Outcomes Spreadsheet (in the attachments) or you may upload something else that you will be using for data collection of qualitative and/or quantitative data for your project. We provided the Data Outcomes as an option to make it easier for organization who might not have a survey, logic model or other means to evaluate their project.

If you do use the Data Outcomes sheet as your evaluation document, please add your name and project title on the top of the form where there is the space to do so. If awarded, your organization will be required to submit the Data Outcomes that is part of the 2013-14 RFA - and not a previous version. If you label and save this document as part of your application it will be ready to go when the 2014 final reports come along!
